The use of Healthengine’s new patient form service is optional. If you proceed, learn how we collect, store, use and disclose data you provide.

The information you provide will be securely stored as a new patient form in our database for 14 days after your appointment date or the form submission date, depending on the practice’s configuration.
We use anonymised data to analyse how people use this product. This helps us improve our products and services.
None of the information you provide will be displayed on the Healthengine website.
By practices
Information you provide will be sent to the relevant health provider for the purpose of administering your health appointment.
The relevant health provider will use the information you provide to create your patient file on their system.
Other people
We do not sell your information to other people or companies.
None of the data you provide will be added by Healthengine to MyHealthRecord.
